Our home care services are provided by highly trained home health care professionals dedicated to the care and comfort of our patients. We are Medicare Certified and Medicaid Certified.
Disease Management
allows the company, through clinical pathways, to manage utilization for patients with chronic diseases. Healthy at Home, Inc. has implemented diabetes, wound care, cardiac, rehabilitation and surgical recovery disease management programs to ensure quality patient care through a individualized plan of care. Healthy at Home, Inc.’s disease management programs focus on utilization of national clinical standards and patient education with a focus on self care management skills.
Diabetes Care.
The Healthy at Home, Inc. Diabetes disease management program was developed based on the national standards set by the American Diabetes Association (ADA). Our nurses closely monitor patients’ blood sugar levels as the doctor orders, administer insulin injections, and report any significant changes immediately. Patients and their families learn basic skills, as well as self-management techniques necessary to control diabetes and the acute and chronic complications associated with it.
Wound Care.
Healthy at Home, Inc. combines its expert clinical and support services to utilize a multidisciplinary team approach led by the physician. Our wound care program provides treatment in a setting most preferred by patients - at home. Standardized wound management protocols based on best practice guidelines promote consistency in practices and outcomes. Our wound care-trained nurses monitor wounds status, change dressings, and administer medication as prescribed.
Surgical Recovery.
Healthy at Home, Inc. has developed a Surgical Recovery program designed to give patients back their functional independence and provides maximum clinical outcomes. Our protocols allow patients to recover at home with the reassurance that their post-operative care needs are being met by highly professional and skilled nurses and therapists in accordance with their physician's orders and recognized standards of care.
Medication Management.
Healthy at Home, Inc has several medication management programs, such as for patients with forgetfulness, such as Alzheimer’s disease and dementia, and for patients on pain medication. Patients receive their medication regimen as their doctor prescribed, and are monitored for effectiveness, and symptoms of complications, med-to-med interactions, and side effects. We can also administer intravenous medications, such as antibiotic therapy.
Improved Strength and Endurance at Home-Physical Therapy, and Occupational Therapy.
Healthy at Home, Inc. has developed a program designed to help with strengthening, mobility and increased independence. After an evaluation by a physical therapist, an individualized program including strength training and transfer exercises is developed for each patient. .In addition, for those patients at high risk for falling, an individualized program including strength training, a home safety inspection and education is developed to prevent falls where they most often occur - in the patient's home. By improving strength, balance, endurance and flexibility, the risk for falls is decreased, and the majority of falls that are deemed preventable are averted.
Personal Care Services.
